Assisted Living Nurse Consultant

<p><strong>The Role</strong></p><p><br></p><p>We’re looking for an Assisted Living Nurse Consultant to own clinical quality and regulatory compliance across our California AL portfolio. You’ll carry equal responsibility for survey readiness and resident outcomes — not one or the other. A working knowledge of the Assisted Living Waiver (ALW) program is required for this role; a significant portion of our residents are ALW participants, and this consultant will be the operational and clinical anchor for that program across assigned communities. This is a field-based role with significant travel across Northern and Southern California.</p><p><br></p><p><strong>What You’ll Own</strong></p><p><br></p><p><strong>Regulatory Compliance & Survey Readiness</strong></p><p>· Ongoing compliance with Title 22 and CDSS regulations across all assigned AL communities</p><p>· Survey preparation, mock inspections, and plan of correction development and follow-through</p><p>· Incident reporting oversight and root cause analysis on high-risk events</p><p>· Ensuring consistent adherence to resident rights, abuse prevention, and reporting requirements</p><p><br></p><p><strong>Assisted Living Waiver (ALW)</strong></p><p>· Serve as the regional clinical lead for ALW program compliance and oversight across assigned communities</p><p>· Ensure accurate and timely level-of-care assessments, Medi-Cal documentation, and managed care coordination</p><p>· Partner with facility teams and health plans to support appropriate resident enrollment, retention, and transitions</p><p>· Identify documentation gaps that put ALW participants at risk and drive corrective action at the site level</p><p><br></p><p><strong>Clinical Quality & Resident Outcomes</strong></p><p>· Monitoring and improving quality indicators across assigned communities — falls, skin integrity, weight loss, behavioral health, and cognitive decline</p><p>· Clinical record reviews to ensure care plans reflect current resident needs and are followed at the floor level</p><p>· Collaboration with memory care teams where applicable to ensure appropriate programming and clinical oversight</p><p>· Identification of at-risk residents and facilitation of timely care transitions when appropriate</p><p><br></p><p><strong>Leadership & Staff Development</strong></p><p>· Direct coaching and development of facility-based DONs and charge nurses</p><p>· Education sessions on Title 22 updates, clinical best practices, and Kalesta standards</p><p>· Building site-level clinical competency that sustains between your visits</p><p>· Onboarding support for new clinical leaders at assigned communities</p><p><br></p><p><strong>What We're Looking For</strong></p><p><br></p><p>Active California Nursing License</p><p>5+ Years in clinical leadership position in the ALF space</p><p>Working knowledge of Title 22</p><p>Experience with ALF and Memory Care</p>
