Overview
We are seeking a compassionate and dedicated Home Care Nurse to provide high-quality care to patients in their homes. The ideal candidate will possess a strong background in nursing, with experience in hospice care, working with individuals with developmental disabilities, and administering care plans. This role requires a commitment to enhancing the quality of life for our patients while ensuring their comfort and dignity.
Responsibilities
- Deliver personalized nursing care to patients in their homes, focusing on their individual needs and preferences.
- Develop and implement comprehensive care plans tailored to each patient’s condition and requirements.
- Monitor vital signs and assess patients’ health status regularly to ensure optimal care.
- Administer medications as prescribed and educate patients and families about medication management.
- Provide support for individuals with autism and other developmental disabilities, ensuring their specific needs are met.
- Advocate for resident rights and ensure that all care provided is respectful and dignified.
- Collaborate with interdisciplinary teams, including social workers, to coordinate comprehensive care for patients.
- Maintain accurate documentation of patient progress, changes in condition, and any interventions performed.
Requirements
- Valid nursing license in the state of practice.
- Experience in hospice care or nursing home settings is preferred.
- Knowledge of care plans and the ability to implement them effectively.
- Familiarity with CPR techniques and emergency response procedures.
- Experience working with individuals with developmental disabilities or autism is a plus.
- Strong communication skills to effectively interact with patients, families, and healthcare teams.
- Ability to demonstrate compassion, patience, and empathy in all interactions with patients.
- Commitment to maintaining patient confidentiality and adhering to ethical standards of practice.
Job Types: Full-time, Part-time, Temporary
Pay: $15.00 - $75.00 per hour
Expected hours: No less than 20 per week
