Nurse Practitioner/Physician Assistant - Home Care
We are seeking a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing home care team. You will be providing high-quality, patient-centered care to elderly patients in the comfort of their homes, focusing primarily on geriatrics.
Responsibilities:
- Conduct home visits to elderly patients and provide comprehensive medical care.
- Perform assessments, develop care plans, prescribe medications, and coordinate with other healthcare providers.
- Provide education and support to patients and their families regarding disease management, medications, and health maintenance.
- Document patient encounters accurately in the EMR.
Qualifications:
- Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in New York State.
- Experience in geriatrics and/or home care preferred but not required.
- Strong communication skills and compassionate bedside manner.
- English and Russian fluency required.
- Ability to work independently with strong organizational skills.
Schedule & Availability:
- Full-time or part-time positions available in South Brooklyn.
- Part-time opportunities available in Staten Island.

Pay: $115,861.00 - $171,717.00 per year
